WEST GARFIELD PARK — A 28-year-old man died and a 17-year-old was wounded following a shooting on the West Side on Monday evening, police said.

The Cook County Medical Examiner's Office identified the man as Cory Hughes, of the 100 block of North Laporte Avenue.

At 8:45 p.m., the 28-year-old was standing on the sidewalk in the 100 block of South Kilpatrick Avenue when someone walked up and shot at him, according to a police incident report. He was brought to Mount Sinai Hospital, where he was treated for multiple gunshot wounds, police said, and was later pronounced dead.

Police said the man was a documented gang member.

At the time of the shooting, a 17-year-old was driving past the scene when he was struck by a bullet in his left ankle, police said.

The teen drove himself to Loretto Hospital, police said, where he was listed as "stable."

No one is in custody, police said.
