HUMBOLDT PARK — Police are seeking help in finding a man with mental health challenges who has gone missing from his Humboldt Park home.

Paul Hernandez, 47, was last seen Tuesday wearing a gray jacket, a black jacket, black sweat pants and worn black boots, according to an alert from the Chicago Police Department.

According to police, Hernandez had been taken by bus to St. Mary’s Hospital, 2233 W. Division St., where he is enrolled in therapy classes. He failed to board the bus Tuesday after class and did not return home. He has not been seen or heard from since that time.

Hernandez suffers from bipolar disease and schizophrenia and has been known to walk for long distances. He knows his home address on the 1300 block of North Ridgeway Ave. and has his home phone number in his wallet, police said.

The 5’11 Hispanic man has brown eyes, gray hair, a medium complexion and weighs about 252 lbs. Residents with any information on Hernandez’s whereabouts can contact the Area North Special Victims Unit at 312-744-8266.
