CHICAGO — A Northwest Side mother is being investigated for neglect in the death of her 11-year-old daughter, authorities said.
Azaybia Haynes, 11, was found dead in her home in the 4200 block of West Augusta Boulevard in West Humboldt Park Thursday morning, according to the Cook County Medical Examiner's Office. She was pronounced dead at 11:02 a.m.
Haynes' mother is being investigated by the Department for Children and Family Services for neglect, said Dave Clarkin, DCFS spokesman.
DCFS has investigated the woman before. The department received a hotline call in March 2012 alleging neglect by the mother, and those allegations were found to be true, Clarkin said.
The family was offered services, Clarkin said, though he couldn't specify what kind or if the family used them.
No other children live in the home where Azaybia was found, Clarkin said.
Police had no information available as of Friday afternoon.
