ROGERS PARK — Heartland Health Centers plans to open its third Rogers Park facility on Devon Avenue.

The one-story facility will replace a long-vacant building at 1300 W. Devon Ave. formerly housing Weinstein Family Services, a funeral home, the Chicago-based health care provider announced.

Gwenn Rausch, Heartland's CEO, said the old building would be demolished to make way for the new facility, which will offer adult primary care, pediatrics, women’s health and behavioral health services.

The facility will also house a call center to support the provider's 10 other locations on the North Side.

In 2011, the organization was awarded a $3 million grant from the state to help build the facility.

Rausch said Heartland struggled to find a good location for the new facility before finally deciding to build on a stretch of Devon Avenue that the company hopes to revitalize.

"This site is ideally suited for serving the residents of Rogers Park and Edgewater," Rausch said in a statement. "It’s also located in an area designated as underserved by the U.S. Department of Health and Human Services."

The designation, she said, qualifies the doctors and nurses who would work there for loan forgiveness benefits.

The facility will employ about 20 staff members.

Construction on the 9,200-square-foot facility is expected to begin in September.
