MANHATTAN — A 12-year-old girl has vanished from her East Harlem home — the fourth time she has gone missing in recent months, authorities said.

Zakiyyah Taylor disappeared from her building on Madison Avenue, near East 112th Street, on Dec. 20, police said.

Taylor vanished most recently on Dec. 5, but was found three days later.  Before that she went missing in October and September.

Police say that the girl is 5-foot-2, 115 pounds, with brown eyes and black hair. She was last seen wearing a black coat, tan sweater and black boots.
