Man Swipes Prescription Pad and Tries to Use it to Get Drugs, DA Says

OLD TOWN — A man swiped a prescription pad from his former nurse and used it to try to get prescription drugs at four Staten Island pharmacies, prosecutors said.

On July 30, Keith Quagliarello, 42, told police he took four or five prescription pads from his nurse after she decided to stop treating him because she thought he was abusing medication, a law enforcement source said.

That same day, Quagliarello went to the CVS at 1361 Hylan Blvd. and used the pad to get 120 alprazolam pills, prosecutors said.

From Aug. 1 to Aug. 4, Quagliarello tried to use the pads to get drugs six other times from various CVS locations around the borough, but was turned away each time, prosecutors said.

He was arrested on Oct. 20 and charged with forgery, criminal possession of a forged instrument, criminal possession of stolen property, fraud and deceit related to controlled substances, criminal possession of a controlled substance and attempted criminal possession of a controlled substance, the Staten Island District Attorney's office said.

Quagliarello was released without bail and is due back in court on Jan. 27, according to online court records.

His lawyer did not immediately respond to a request for comment.
