UPPER WEST SIDE — CityMD, the walk-in health care provider, is moving its Broadway and West 91st Street clinic to Broadway and West 88th Street, its CEO said.

The new location will take over the space that formerly housed steakhouse Aged, which closed in April, said CityMD CEO Dr. Nedal Shami.

"This new facility will have more patient exam rooms, an updated design consistent with the CityMD brand, and upgraded X-Ray/IT equipment," Shami wrote in an email.

The new location is set to open on Dec. 2, he said.

Jaros Pharmacy, at 25 Central Park West at West 63rd Street, is closing "in the next month or so," said a representative from Winick Realty Group, which is marketing the 1,467-square-foot space.

The pharmacy is moving to West 62nd and Central Park West, the representative said.

The owner of the longtime pharmacy, which residents said had been in the neighborhood since the '80s, was not available for comment.

Lansky's, the old world Jewish deli on Columbus Avenue that closed in February, will become home "a two-star restaurant that is currently in SoHo," Adler said.

But Rafe Evans, who is brokering the deal and also would not share the restaurant's name, said it's not a done deal.

The 2,814-square-foot space, between West 70th and 71st streets, is renting for $59,000 a month.

Editors Note: Our original report saying that luxury women's clothing store Calypso St. Barth was moving into the former Bang & Olufsen space at 330 Columbus Ave. was innacurate. The deal apparently fell through, according to the property's broker.
